Quantum-Functional Intake Call

Service Description

Your health journey with us begins with this information gathering call with collaborative care team member Georgina Martin, RN. In this visit Georgina will review the medical history provided on your intake forms and get to know more about you, your story, and your goals and expectations for treatment. At the conclusion of the call Georgina will share Dr. Maysa’s wellness notes with you so that you can begin implementing gentle lifestyle recommendations while you await your in person visit with Dr. Maysa and Georgina.

Cancellation Policy

To cancel or reschedule you must text 929-DR-MAYSA at least 24 hours in advance of your visit start time to avoid a “no show” fee equal to 50% of the visit cost.
